The National Hospice Foundation’s (NHF) Legacy Society was created to honor and recognize the generosity of forward thinking individuals who have made the important choice to support the mission of NHF through their estate plan.
Membership in the Legacy Society is extended to individuals who have shared information with us that they have made future provisions for NHF through their will or living trust, designation as beneficiary of a retirement plan or life insurance policy, or by making a life income gift (such as charitable gift annuity, charitable remainder trust, or charitable lead trust).
To learn more about the role you can play in securing the future for National Hospice Foundation and join with others who share their compassionate vision for ensuring that quality end-of-life care will always be available, please contact Sarah Meltzer in our gift planning office or call (877) 470-6472.
Membership is voluntary and without obligation.
If you have included the National Hospice Foundation in your estate plans, we invite you to print, complete and return this form so that we may properly recognize your gift in the Legacy Society.
