NHF welcomes gifts paid by check and sent through the mail.

Please make checks payable to either the National Hospice Foundation or the FHSSA Impact Fund and mail them to:

If your own name is not printed on the check, please write it on the check so that we may send you the appropriate thank you letter that you can also use for tax purposes.

If you are making a memorial or "in honor" contribution, please

1) tell us the name of the person you are memorializing or honoring

2) give us the name and address of the person you would like to receive an acknowledgment letter.
